Adult U.S. Passport Renewal

Please bring the original of the following documents with you to your appointment:

  • U.S. Birth Certificate, Consular Report of Birth Abroad, Certificate of Naturalization, or Certificate of Citizenship OR your current passport.
  • Valid photo ID (e.g. valid passport or two other government-issued photo ID, such as driver license, State ID card, voter’s ID, etc.)
  • DS-11 form
  • One 2″x 2″ photo
  • Printout of appointment confirmation
  • For DS-82:  $110 in cash (U.S. dollars or Ugandan shillings equivalent) or credit card

First Time Minor U.S. Passport or Renewal (15 years old or under)

Please bring the following documents to your appointment:

  • Proof of U.S. citizenship, which may be any of the following: U.S. Birth Certificate, Consular Report of Birth Abroad, Certificate of Naturalization, or Certificate of Citizenship
  • Proof of Parentage listing the names of the child’s parents (e.g. Long-Form Ugandan birth certificate issued by the Ugandan Registration Services Bureau, U.S. birth certificate or Consular Report of Birth Abroad)
  • DS-11 form (completed but not signed)
  • DS-3053 form (if applicable) or primary evidence of sole authority to apply for the child’s passport e.g. court order, death certificate
  • Printout of appointment confirmation
  • Sequential or “growing up” photos for the applicant that cover the time between the last passport and the present.
  • Valid photo I.D. of the child’s parents/guardian (e.g. valid passport or two other government-issued photo ID, such as driver license, State ID card, voter’s ID, etc.)
  • One 2″x2″ photo
  • $105 in cash (U.S. dollars or Ugandan shillings equivalent) or credit card

Effective April 2, 2018, the passport execution fee will increase from $25 to $35.  The $10 execution fee increase applies to U.S. passport applicants using the DS-11 form including: first-time applicants over age 16, children under age 16, and applicants who reapply after reporting their previous passport lost or stolen.

Customers applying with the DS-11 form at a U.S. embassy or consulate abroad pay two separate fees: an application fee to the U.S. Department of State and the execution fee to the Passport Acceptance Facility (such as a post office in the United States or a U.S. embassy or consulate abroad).  Accordingly, the total passport fee for such applicants will increase by $10 in total.

The $10 fee increase does not apply to adults eligible to renew their passport using the DS-82 form.
