Passport Information
You may apply for a U.S. passport if you are already in possession of one of the following documents:
- U.S. birth certificate
- Consular Report of Birth Abroad
- Certificate of Naturalization
- Certificate of Citizenship
Step 1:
Fill out electronically and print the DS-11 Application for a U.S. passport wizard form. If you experience trouble with the Passport Application Wizard, click here for the Form DS-11 (PDF 100 KB). To view PDF files, you need a PDF reader such as Adobe Acrobat. Get a free download from the Adobe website.
- Check that you have read the Privacy and Computer Fraud and Abuse Acts Notices and Disclaims and click submit.
- Click Apply Online
- Print the DS-11 form upon completion.
- Please note that some instructions and options on the form are only applicable in the United States.

Step 3:
Both parents must provide consent authorizing passport issuance for a minor under age 16. See the scenarios below, and follow the instruction that best applies to your circumstance:
Both Parents MUST:
- Appear in person with the minor
- Sign Form DS-11 in front of a Consular Officer
One Parent MUST:
- Appear in person with the minor
- Sign Form DS-11 in front of a Consular Officer
- Submit the second parents’ notarized Statement of Consent (Form DS-3053). This form must be notarized by a U.S. commissioned notary or a Consular Officer of a U.S. Embassy or Consulate.
One Parent (with sole legal custody) MUST:
- Appear in person with the minor
- Sign Form DS-11 in front of a Consular Officer
- Submit primary evidence of sole authority to apply for the child with one of the following:
- Minor’s certified U.S. or foreign birth certificate listing only the applying parent
- Consular Report of Birth Abroad (Form DS-240) or Certification of Birth Abroad (Form DS-1350) listing only the applying parent
- Court order granting sole custody to the applying parent (unless child’s travel is restricted by that order)
- Adoption decree (if applying parents is sole adopting parent)
- Court order specifically permitting applying parent’s or guardian’s travel with the child
- Judicial declaration of incompetence of non-applying parent
- Death certificate of non-applying parent
NOTE: If none of the above documentation is available, the applying parent must submit Form DS-5525 (PDF 41 KB) stating why the non-applying parent/guardian’s consent cannot be obtained. The form must be duly notarized by a U.S. commissioned notary or by a Consular Officer of a U.S. Embassy or Consulate.
A Third Party (in Loco Parentis applying on behalf of a minor under the age of 16) MUST:
- Statement of Special/Exigent Circumstances Form DS-5525 (PDF 41 KB), duly notarized by a U.S. commissioned notary or by a Consular Officer of a U.S. Embassy or Consulate.
- Submit a notarized written statement or affidavit/Special Power of Attorney (SPA) from both parents giving their consent to the issuance of the applicant’s U.S. passport and authorizing a third-party to act in the parents’ capacity. The SPA should contain the applicant’s full name and date of birth. The document must be duly notarized by a U.S. commissioned notary or a Consular Officer of a U.S. Embassy or Consulate.
- When the statement of affidavit is from only one parent/guardian, the third-party must present evidence of sole custody of the authorizing parent/guardian.
IMPORTANT NOTE: The written consent (Form DS-3053) from the non-applying parent that accompanies an application for a passport for a minor under the age of 16 must be less than 3 months old. New written consent from the non-applying parent must be obtained and submitted with any future passport application for the minor under age 16.
Step 4:
Please bring the following documents to your appointment:
- Proof of U.S. citizenship, which may be any of the following: U.S. Birth Certificate, Consular Report of Birth Abroad, Certificate of Naturalization, or Certificate of Citizenship
- Proof of Parentage listing the names of the child’s parents (e.g. Long-Form Ugandan birth certificate issued by the Ugandan Registration Services Bureau, U.S. birth certificate or Consular Report of Birth Abroad)
- DS-11 form (completed but not signed)
- DS-3053 form (if applicable) or primary evidence of sole authority to apply for the child’s passport e.g. court order, death certificate
- Printout of appointment confirmation
- Sequential or “growing up” photos for the applicant that cover the time between the last passport and the present.
- Valid photo I.D. of the child’s parents/guardian (e.g. valid passport or two other governement-issued photo ID, such as driver license, State ID card, voter’s ID, etc.)
- One 2″x2″ photo
- $105 in cash (U.S. dollars or Ugandan shillings equivalent) or credit card
Step 5:
Once payment has been made and application has been approved, passport will be processed within 2 weeks.
